Community Support
Forward House’s three teams of mental health workers provide assistance designed to respond appropriately to each individual’s specific needs and life situation. Flexibility and mobility are crucial elements of these three services which tend to focus upon basic life management issues such as social assistance benefits, medication, appropriate housing, nutrition, physical health, budgeting, shopping, individual rights and family concerns, among others.


Long-term Support
Long- term Support staff are mandated to respond to each individual’s expressed and/or identified needs. The services they provide are designed to assist individuals in maintaining their independence in the community

Clients work in collaboration with their assigned staff member to define the aims and objectives they wish to achieve. This client-centered approach allows them to progress at their own pace while gradually developing their potential and level of integration in the community.

Intensive Support
​
This team assists individuals who are striving to cope with severe and persistent mental illness, overseeing their long-term psychosocial and psychiatric welfare. Team members work together as a whole, sharing caseloads and providing service tailored to each individual's special needs and adapting to their specific situation. Service delivery plans can be updated every 48 to 72 hours and are frequently punctuated with crisis interventions.

Most individuals receive services at least once every week, and when medication compliance is a primary concern, services are provided daily, Monday to Friday. As the issues are often complex, the team works in collaboration with the individual's hospital-based treatment providers.
